🩺How Cervical Spine Manipulation May Reduce Pain in People With Degenerative Cervical Radiculopathy

Neck pain that travels into the shoulder, arm, or hand can be frustrating—especially when degenerative changes in the cervical spine are involved. This pattern of symptoms is commonly associated with cervical radiculopathy, which occurs when a cervical nerve root becomes irritated or compressed.

For carefully selected patients, conservative care may include exercise, education, manual therapy, and—in appropriate circumstances—cervical spine manipulation. Research suggests that certain manual therapy approaches may help reduce pain and improve function in people with cervical radiculopathy, although the evidence specifically supporting cervical manipulation remains limited and mixed.

What Is Degenerative Cervical Radiculopathy?

Degenerative cervical radiculopathy develops when age-related or degenerative changes affect structures around the cervical spine. Disc degeneration, disc bulging or herniation, and changes involving the facet joints or uncovertebral joints can contribute to narrowing around a nerve root.

When a nerve root becomes irritated, symptoms may include:

  • Neck pain or stiffness
  • Pain extending into the shoulder, arm, or hand
  • Numbness or tingling
  • Changes in sensation
  • Weakness in certain muscles
  • Difficulty with neck movement or everyday activities

Importantly, not everyone with degenerative changes on an X-ray or MRI will have symptoms. A comprehensive clinical evaluation is necessary to determine whether the patient’s symptoms are consistent with cervical radiculopathy.

How Could Cervical Manipulation Help Reduce Pain?

Cervical spine manipulation does not “put a disc back in place” or reverse degenerative changes. Rather, the potential benefit is thought to involve several interacting effects on the musculoskeletal and nervous systems.

1. It May Improve Cervical Joint Mobility

Degenerative changes can be associated with stiffness and reduced movement in portions of the cervical spine. Carefully applied manipulation or mobilization can temporarily improve joint motion and reduce mechanical restriction.

Improved movement may make it easier for a patient to perform normal activities and therapeutic exercises without provoking as much discomfort.

Research examining manual therapy for cervical radiculopathy has found improvements in pain and disability, with cervical and thoracic mobilization/manipulation among the approaches supported by moderate-confidence evidence in an umbrella review.

2. It May Influence Pain Processing

One potential mechanism of spinal manipulation is a neurophysiological response rather than simply a mechanical effect on a joint.

Manual therapy can stimulate sensory receptors in the spine and surrounding tissues. This sensory input may temporarily change how the nervous system processes and modulates pain.

In practical terms, the goal is not simply to “move a joint,” but potentially to reduce sensitivity and make movement more comfortable.

3. It May Reduce Protective Muscle Guarding

When the neck is painful, surrounding muscles may become tense or guarded. That protective response can further restrict movement and make ordinary activities uncomfortable.

Reducing stiffness and improving movement may help decrease some of that guarding, allowing the patient to move the cervical spine more normally.

This can be particularly useful when manual therapy is combined with active rehabilitation rather than used as a stand-alone treatment.

4. It May Make Exercise and Movement Easier

One of the most important goals of conservative care is helping a patient return to comfortable movement and normal function.

Manual therapy may provide enough short-term symptom relief to make therapeutic exercise, postural changes, and activity modification easier to perform.

Current evidence generally favors multimodal care, rather than relying on manipulation alone. Manual therapy combined with exercise and other conservative strategies is supported for several neck pain presentations, while evidence specifically addressing degenerative cervical radiculopathy continues to evolve.

What Does the Research Say?

The evidence is encouraging in some areas but should be interpreted carefully.

An earlier systematic review of three randomized trials involving 502 people with degenerative cervical radiculopathy found moderate-quality evidence that cervical manipulation produced an immediate reduction in pain compared with cervical computer traction. However, the authors noted that safety conclusions were limited because adverse-event reporting was inadequate.

More recent research has broadened the evidence surrounding manual therapy. A systematic review and network meta-analysis published in 2025 found that manual therapy was associated with improvements in neck pain and disability in patients with cervical radiculopathy, although the authors emphasized the need for additional high-quality trials.

An umbrella review of systematic reviews similarly found moderate-confidence support for cervical or thoracic mobilization/manipulation, neural mobilization, distraction, and soft-tissue treatment in cervical radiculopathy, particularly for short-term improvements in pain and disability.

A 2025 clinical practice guideline also recommended manual therapy for cervical radiculopathy, including evidence showing improvements in pain and cervical function.

At the same time, research does not establish that cervical manipulation is appropriate for every person with cervical radiculopathy. Treatment should be individualized based on the patient’s symptoms, neurological findings, medical history, and clinical presentation.

Is Cervical Manipulation Appropriate for Everyone With Cervical Radiculopathy?

No.

This is an important distinction.

Cervical manipulation should not be viewed as a routine treatment for everyone with radiating neck or arm pain. Patients with progressive neurological deficits, significant weakness, signs of spinal cord involvement, or other concerning findings require appropriate medical evaluation.

Clinical guidance has specifically cautioned against manipulation when acute or progressive neurological deficits are present.

A careful examination helps determine whether conservative chiropractic care and manual therapy are appropriate—or whether referral for additional medical evaluation or imaging is warranted.

Why an Individualized Approach Matters

Degenerative cervical radiculopathy can look very different from one person to another.

One patient may primarily experience neck stiffness and intermittent arm discomfort. Another may have significant numbness, weakness, or persistent nerve-related symptoms.

For that reason, treatment should be based on the individual rather than simply the diagnosis shown on an imaging report.

A conservative care plan may incorporate:

Chiropractic manipulation or mobilization: When clinically appropriate, selected techniques may be used to address mobility restrictions.

Manual therapy: Soft-tissue work and other hands-on techniques may be used to address surrounding muscular and joint restrictions.

Therapeutic exercise: Strengthening, mobility, and stabilization exercises can help patients improve movement and function.

Activity modification: Temporarily adjusting aggravating activities can help calm symptoms while maintaining appropriate movement.

Patient education: Understanding the condition and learning how to manage symptoms can be an important part of long-term recovery.

The Goal Is Better Function—not Just Less Pain

Pain reduction is important, but successful conservative care should also focus on helping a person move, work, sleep, exercise, and participate in daily activities more comfortably.

Cervical manipulation may be one component of that process for an appropriately evaluated patient. It is not a treatment that reverses arthritis, restores worn cartilage, or permanently “fixes” degenerative changes.

The goal is to improve mobility, comfort, and function while helping the patient return to normal activity as safely as possible.

When Should You Seek Further Evaluation?

Radiating neck pain should not automatically be assumed to be a simple muscle problem.

Medical evaluation is particularly important when symptoms involve:

  • Increasing or progressive weakness
  • Significant loss of coordination or dexterity
  • Problems with balance or walking
  • Severe or rapidly worsening symptoms
  • New bowel or bladder dysfunction
  • Other neurological changes that are unusual or unexplained

These findings may indicate a condition requiring more urgent evaluation rather than routine conservative treatment.

Takeaway

For some people with degenerative cervical radiculopathy, carefully selected cervical spine manipulation or other manual therapy may help reduce pain and improve mobility. Research supports manual therapy as part of conservative management, but the evidence for cervical manipulation specifically remains less definitive than the evidence for multimodal care overall.
